1. Information We Collect

We collect only the information you voluntarily provide through contact forms, booking requests, or email communications. This may include your name, email address, phone number, and brief message details. We do not collect sensitive health information through our public website — clinical intake happens through our secure, HIPAA-aligned booking portal.

2. Cookies & Tracking

We use cookies to understand site performance and improve your experience. You may choose which categories to allow via our cookie banner:

  • Necessary Cookies: Always active — required for site functionality and security.

  • Analytics Cookies: With your consent — help us understand which pages are visited and how visitors navigate the site. Data is anonymized where possible.

  • Marketing Cookies: With your consent — used to show relevant content and measure ad performance.

3. How We Use Your Information

Your information is used solely to respond to your inquiry, schedule appointments, and provide requested information about our services. We do not sell, rent, or trade your personal information. We do not share your data with third parties for their own marketing purposes.

4. Data Storage & Security

We implement industry-standard security measures to protect your information. Data is stored on secure, encrypted servers. Communications sent via contact forms are encrypted in transit.

5. Your Rights — "Do Not Use My Information"

You may request at any time that we stop using your information for analytics, marketing, or non-essential communications. To do so, email us at privacy@empezarwellness.com with the subject line: "Do Not Use My Information". We will comply within 7 business days.

6. Request Deletion

You may request full deletion of any personal data you have shared with us. Email privacy@empezarwellness.com with: "Delete My Data" in the subject line. We will verify your identity and remove your data unless retention is required by law.

7. HIPAA & Clinical Data

Protected health information (PHI) is collected and managed exclusively through our secure Sessions Health portal, which operates independently and complies with HIPAA and applicable Florida regulations. Information submitted through this public website is not used for clinical treatment records.
